Being well prepared for your appointment will ensure that the doctor has all the information needed to provide you with the best possible care.
For your convenience, we’ve provided you with some information you’ll need for your first visit to our office. Feel free to print and fill out the New Patient Information Form and review The Foot & Ankle Patient Privacy Notice prior to your appointment. We also have this information available at our offices. If you choose to review these forms at one of our locations, please plan to come 10 minutes prior to your scheduled appointment as you’ll need time to complete the paperwork.
In addition, it is important to inform your doctor of any medications you are currently taking. If you are taking several types of medications you may want to bring a list.
And finally, if you plan on using insurance as your primary form of payment, be sure to bring your card with you! All this will help to ensure a smooth office visit.
